  • Standard Benefit Plan Designs
    Employers choose to offer 3 Limited Day Minimum Value Enhanced Plans (MVP) (Bronze Plus, Silver, and Gold
  • Employer Billing/Invoices

    Invoices are sent via email to the client’s billing email address. Monthly Plan fees are standardly remitted via ACH on or about the 1st business day of each month.

    ID Cards

    Enrolled participants will receive a Health Plan ID card from the TPA. The ID card will include information on the Plan’s Medical and Pharmacy coverage

  • Mobile App Support Tool

    Access to Plan information, services and features, including digital ID cards and concierge support, are all in one place and at Participants’ fingertips. Whether it’s to find a doctor, check a claim, track progress toward out-of-pocket maximums or just keep up with important benefit messages, Plan participants are encouraged to download the app from Apple Store or Google Play Store.

  • Common Questions
    • What employees can a Plan sponsor deem eligible for coverage?
      Full-time, Part-time staff, hourly, salaried, seasonal, and union
    • When do dependents terminate coverage after turning age 26?
      Dependent coverage will terminate at the end of the month in which the dependent turns age 26, unless the law requires an extension due to disability, etc.
    • Can I get diagnostic tests (lab, x-ray, complex imaging)?
      No, outpatient diagnostic services must be done in an office or freestanding facility. Non-emergent imaging services rendered in a hospital setting are not covered.
    • Does my plan cover dependent maternity benefits?
      No, maternity services for dependent children are not covered.
    • Does any of the HBA plan designs cover infertility treatment?
      No, Infertility treatment is not a covered service.
    • What should I do if my claim is denied because the physician's office is owned by a hospital system?
      Call your Customer Service Team and explain the situation. Elective diagnostic tests are covered at standalone facilities but not covered at a hospital.
    • Will non-US citizens be able to secure coverage?
      Yes, Employees with a Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN), may be eligible for HBA plan designs.
    • Will my plan cover treatment of procedures to change my physical anatomy to that of the opposite sex?
      No, the plan does not provide coverage for these procedures.
    • Does my plan cover the treatment for sexual dysfunction?
      No, treatment for sexual dysfunction is not a covered service.
    • Will my plan cover service for diagnosis or treatment for sleep apnea?
      No, the plan does not cover the diagnosis and treatment of sleep apnea, including CPAP machines.
    • Does my plan cover Private Duty Nursing or Skilled Nursing facilities?
      No, there is no coverage for Private Duty Nursing or Skilled Nursing facilities.
